How To Register

If you live within the practice area (boundaries defined below) complete a registration form. Newly registered patients including children will be offered a nurses appointment for a health screen.

Practice Boundaries

To the north
A457 Tipton Road / Sedgley Road as far as junction with A4123.
To the east
A4123 Birmingham New Road, as far as junction with A4037 Tipton Road. A4037 to Castle Gate Island
To the south
A461 Dudley By-Pass as far as Scotts Green Island. A4101 Kingswinford Road as far as Tansey Green Road
To the west
Tansey Green Road then right into Stallings Lane, left into Guys Lane then left into Himley Road, right into Brick Kiln Lane then left into Eaton Crescent, Left into Cotwall End Road then right into Catholic Lane, right into Gate Street, then right onto A457 to the north.

In exceptional circumstances we may consider registration of patients outside of the practice area.  

This would include:
on the North / West border up to and including Sandyfields Road, up to Gospel End Road / Gospel End Street

on the South on Holly Hall, bordered by Peartree Lane and Pedmore Road, Cochrane Road and Stourbridge Road.

Change of Personal Details

Please let us know as soon as you change your name, address or telephone number.

Complete a form available from reception and ensure all family members affected by the change are entered onto the form. Computer records are NOT updated automatically unless you notify us of each family member's change of details.

Since mobile phones are more popular, many patients no longer have a landline number. Please let us know your number so we could contact you if needed.
